Whether you’re submitting electronically or downloading and returning a paper form, this page helps keep matters simple and organised.
Digital notifications
Parents and carers can use our easy online notifications to advise the school of:
Student absence
Unfortunately, partial day absences cannot be reported online.
Late arrival: If a student arrives late, they should report to the school office to collect a late note before attending class.
Early departure: If a student needs to leave early, parents should visit the school office to collect an early leaving slip before collecting their child. Office staff will arrange for the child to be brought to the office.
Teacher meeting request
Your child’s class teacher is the best first point of contact for any questions or concerns about their learning or wellbeing.
The preferred way to contact your child’s teacher is by email, whether you’d like to ask a question, request a phone call, or arrange a meeting. If you don’t have your child’s teacher’s email address, please send your message to the school office, and it will be forwarded on.
Alternatively, you can submit a Teacher Meeting Request form.
Change of details
Use this form to update your details at the school.
For example; change of address, contact phone number or emergency contact.
Kiss & Go registration
Parking around the school is not always easy at drop-off and pick-up. As a courtesy, the school provides a Kiss & Go service.
Families wishing to use the Kiss & Go service first need to register here.
Paper notifications
A few notifications still need to be provided on paper. You can download the forms using the links below, then hand the completed forms in at the school office.
Transition to School Program: Information Form - New Students
Please complete this form and bring it with you on the day your child starts school.
This information will help the school and your child’s teacher to get to know your child and cater for his or her needs.
Application for Extended Leave - Travel
If your child will be absent from school for 5 days or more outside of the scheduled vacation periods, please complete this form and provide a copy of your travel itinerary or flight details (if applicable).
This form applies to all extended absences, including those for family holidays, travel, or bereavement.
Publication of Student Images Consent Form
Schools like to document, celebrate and promote the learning journey, achievements and experiences of their students using photos and videos. These images are taken throughout the year in classrooms, on excursions and at other school events.
The school requires parent's/carer's written permission to publish your child’s image.
SAKG Media Permission Form
Our school is a proud participant of the Stephanie Alexander Kitchen Garden Program.
The Stephanie Alexander Kitchen Garden Foundation requires permission for my child to be interviewed, photographed, or filmed by Foundation staff, the media, or their authorised representatives.
Images or footage may be used by the media or in future promotional materials for the Foundation.
Students Requiring Prescribed Medication Form
Parents must advise the school of any medical condition that affects their child and requires prescribed medication during school time.
Parents should complete the Prescribed Medication Form, which allows the school to administer prescribed medication.
Costs – Application for Waiver/Partial Waiver Form
Stanmore Public School offers a range of programs and activities that enrich our students’ learning beyond what is covered by government funding. These are open to all students, and we encourage everyone to take part.
Families are asked to reimburse the school for these costs. If you’re experiencing financial difficulty, please contact the office to discuss a payment plan or partial fee waiver.
